ENROLLMENT
Please fill out the application and return with the required documentation as detailed below. You may email to mhandy@fulmont.org or drop off at one of our locations. For questions or to begin the enrollment process via telephone, please call 518-775-5754 ext 7057.

Please be aware that the following items must be submitted before your child can be accepted:
1. Proof of Annual Income
o Tax return
o Last pay stubs of the year outlining your year to date salary
o Employer statements on letter head outlining your annual income for the previous year
o TANF printout
o SSI printout
o Disability printout
o SNAP(food stamps) printout
o Social Security
o Unemployment printout
o Child support (either court order or notarized letter stating amount received)
o Rental income
o If you were not employed, you must submit a notarized letter stating the time period you were not employed.

3. Copy of Child’s Immunization Record
ELIGIBILITY
Children from age three to age five from families with low income, according to the Poverty Guidelines published by the Federal government, are eligible for Head Start services.
Children in foster care, homeless children, and children from families receiving public assistance (Temporary Assistance for Needy Families or Supplemental Security Income) are also eligible for Head Start services regardless of income.
Head Start programs may enroll children from families that have incomes above the Poverty Guidelines.
If there is not a space for your child at this time, ask the program to put your family on the waiting list.
